Physical Therapy for Parkinson’s: What LSVT BIG and LOUD Actually Involve

Parkinson’s disease affects movement and voice gradually. LSVT BIG and LOUD are two specialized, structured programs designed specifically to address those changes.
What’s happening with movement. Movements gradually become smaller, handwriting gets smaller, and speech becomes quieter without the person always realizing it. This is sometimes called bradykinesia and contributes to falls, communication difficulty, and reduced independence.
What LSVT BIG involves. LSVT BIG is an intensive physical and occupational therapy program that retrains bigger, intentional movements across walking, reaching, turning, and daily activities. It is often delivered four sessions a week for four weeks, with high-effort, high-repetition practice.
What LSVT LOUD involves. LSVT LOUD is the speech-focused counterpart, addressing quieter or mumbled speech. It retrains a louder, clearer voice, which improves communication, safety, and confidence.
Why intensity matters. Both programs use intensive, high-repetition practice to drive the neuroplastic changes needed for movement and speech patterns to genuinely shift, not just improve temporarily during a session.
Patients and families may notice improvements in daily function, walking, balance, communication, confidence, and independence. Results vary by individual and disease stage, but a certified therapist can help determine whether the timing and program are a good fit.
